The Welsh Goverment Regional Integration Fund is a programme in Wales that helps make health and social care services work better together. The fund will run for 5 years, from April 2022 to March 2027.

It aims to create new ways of providing care that are better for everyone. This includes helping:

• Older people
• People with Dementia
• Unpaid Carers, including Young Carers
• Children with additional support needs
• People with mental health needs
• People with Learning Disabilities and Neurodevelopmental conditions

By the end of the five years, we hope to have:

• Established the six National models of integrated care in Gwent.
• Made sure people have a good experience of using services.
• Continued to develop health and social care services for the better.
• Made these new ways of working a regular part of services across Gwent.
